But often they are not as strong as you might think. They need lots of support. For instance, they may now be at high risk for falling. Or they may have new medications that will take a while to get used to. Maybe there are exercises they need to do every day. Or they may need help with personal care such as bathing and dressing until they have fully recovered.

Meet with the discharge planner
The sooner we meet with you and your family, the better.
Once you have decided you want us for your home care needs, we’re happy to come to the hospital before discharge and meet with the discharge planner. We can go over the discharge plan to be sure we understand what the caregivers need to do.
If you are not able to drive your loved one home, we will gladly do that. Also, get them set up at home so everything is conveniently where they need it. We’ll even stop by the pharmacy to pick up any new medications that have been added by the hospitalist doctor.

Set up home care
Our job is to make sure the home is ready to support your family member’s full recovery.
Whether they are coming home from the hospital or from a skilled nursing facility, we will walk through your loved one’s daily routines. We’ll review the discharge planning notes and make sure everything that has been ordered has been delivered and is ready to use.
Together we’ll determine how many hours of home care you’ll need during the day. With our flexible scheduling and no minimum days per week, we can arrange for two-hour or three-hour short shifts a couple days per week, up to twenty-four-hour care for those who need more intensive support.
Technology and durable medical equipment. Because our team has so many years of experience, we know how to operate and monitor common devices. From hospital beds to ambulation equipment, our home caregivers are trained to support your family member’s recovery. And if we notice that something would be helpful (bedside commode, guard rail to prevent rolling out of bed, Hoyer lift for those who are bedbound), we can make recommendations and help you secure the durable medical equipment and supplies you need.
Collaboration with therapists. Did your loved one fall and break a hip? Maybe they need regular visits from a physical therapist, with exercises to perform each day to get stronger. Perhaps your relative had a stroke and needs help with swallowing exercises so they don’t choke, or with cuing to swallow with their head at a particular angle. Our in-home caregivers can be present during therapist visits so they can ask questions and then help implement the plans over the course of the day. Plus, our caregivers are trained in CPR, which includes passing the test for the Heimlich maneuver if your family member were to swallow some food incorrectly and start choking.

For those with no one local to help
- Are you caring for parents long distance? We understand how hard that can be. You have responsibilities you need to fulfill. You want to help your loved one, but the distance is challenging.
- Maybe you are a care manager or guardian with a client who does not have adult children to help (a solo ager).
Let us provide the support you need so you can be confident that the person you care for will have the support they need until they are fully recovered. With our flexible staffing arrangements, we can provide twenty-four-hour care when the need is greatest after hospital discharge. Then, as strength and health are regained, we can taper off, eventually providing short shift support of two and three hours.
Plus, our software allows you to see uploaded photos and to read caregiver notes so you can stay abreast of how the recovery is unfolding.
